This information will be used to prepare the 2017 Statistical Report of Second Presbyterian Church to the PCUSA, and to populate a confidential Church Management Information System (CHMS) for Second PC. Access to the CHMS will be closely controlled and monitored to ensure access is only by those authorized.
This form will be sent to the Clerk of Session, George Huggins.
